| The Carlos Otis Stratton Mountain Medical Clinic announces
a new initiative to better serve your family - IN CASE OF EMERGENCY - ICE.
ICE is a program that offers you the opportunity to give us information on your family so that we may CONTACT you in the event of an emergency. Please follow the simple steps below to participate. |

| 1. Complete the information CARD for each member of your family who you would like to register. Cards are available at the Clinic. | ||||||||||||||
| 2. Bring your information and your ski pass or passes to the Clinic any ski day between 9:00 and 11:00 am. | ||||||||||||||
| 3. Turn in your form and have your ski pass tagged so that we can immediately identify those who are registered. | ||||||||||||||
| The Clinic is an independent nonprofit organization that provides emergency medical services and scheduled appointments to the Stratton Mountain community. Our physicians serve as volunteers. Your charitable donations help to underwrite the services we provide to the Stratton Community. All contributions are appreciated and can be sent to: | ||||||||||||||
